Pre-Authorization Remittance (PAR) Form & Envelopes
On the registration form, you can request envelopes from the parish or obtain a form for the Pre-Authorization Remittance (PAR) of your financial contribution. Please note that the registration form and the PAR form can be left in the weekly collection. Completed PAR forms can also be dropped off in the parish office, Monday through Friday, 8:30 am to 4:30 pm.
